Billing and Insurance

When patients have services in our outpatient offices, our staff will collect all medical insurance information at the time of service.  Please bring your insurance ID card(s), as well as a picture ID, to your appointment.  Click here to see a list of which Insurance Providers are accepted at each of our facilities.

If a patient has no insurance coverage for the services to be provided and consideration is being made for paying for the study using our Cash Fee Schedule, please click here.

Request Medical Records

If you need your medical records, simply visit the Request Medical Records page, fill out the form, and one of our staff members will call you within one business day.  Please note, the patient will need to complete an Authorization for Release of Medical Records form and submit to our offices prior to our making the records available.
