Request Medical Records
To request a copy of medical records, please call or email the facility that performed the exams. Requests take 5-7 business days to process and are available for pickup, Monday – Friday from 8 AM – 5 PM, excluding holidays.
An Authorization for Release of Medical Records must be signed and delivered (in person or by fax) before records may be released. This form may be downloaded by clicking here.
